I am 99% sure this was a giveaway promo card. Just throwing it out there in case anyone knows any more info.

Seems like promo card might be correct, according to this article from Sports Business Journal (July 1, 2002), "Rice to tout Spalding footballs".

"Spalding has signed 12-time Pro Bowl receiver Jerry Rice to a two-year endorsement deal. Rice will help market Spalding's new footballs, which for the first time include the company's Infusion technology ... Also supporting the launch will be a Jerry Rice trading card promotion. SFX Sports handles Rice."
