All lists indicate this card does not exist. Here is proof it does exist. Back in 1996, I was unaware Salmon hadn’t hit a home run during the applicable timeframe, so sent in his redemption card. Collectors Choice, nonetheless, fulfilled the request. I am unaware of anyone else having this card.

I have heard of Upper Deck doing this in racing as well, seems they had the cards printed up in advance, and would just fulfill what was sent, regardless of it the criteria to win the card actually happened.  Waiting to get them to confirm, but may be adding some cards to some racing sets with this same issue.
