Is there a reason why this variation is not listed here? I see that the Witt is so what do I need to do to request its inclusion?

Well, the reason it isn't listed is because there didn't seem to be anyone who knew it existed. If there is one you'd just have to submit an IR to get it added. Wouldn't be surprised if visual confirmation was requested.
Which also begs the question. Was this an issued card in the factory set or was it backdoored? If backdoored you may not see it allowed to be entered.

This card was most definitely found in packs, though, not intentionally. The black box was meant to make it easier to locate to pull it from production before pack out.
The Witt card was found in packs and in an 800ct box of backdoored copies. To date, just a few of the checklist have been found in packs.
No copies of either were found in factory sets.

If you are referring to card #702 Mike Witt with the black box, that card is accounted for in here

I thought you were referring to Mike Witt showing up on the Checklist card #800

This is true. There have only been a few sales/offerings since I began tracking it (2003ish) but they are confirmed to come from packs as confirmed by old hobby magazine letter columns and, more recently, about a decade ago by Lee (on ebay/forums as "leema"), who opened a pallet of 1990 hi series product and found several Witts and IIRC one or two of the checklists. FYI, he has a great site on early UD oddities and other rare prototypes: https://rarebaseballcards.myfreesites.net/
Given the major discovery of 800 #702 Mike Witt cards, it is possible that more could turn up.

Just curious why the separate section and aftermarket notation for these variations?
2006 Topps Alex Gordon, 2000 UD Ovation Ryan Anderson and 2000 UD MVP Lavar Arrington all appear as part fo the regular checklist and the "errors and variations" sub sections.

That's where they were originally entered years ago. It's only been in the last year or so that we've moved these types of pulled cards from base to Inserts/Related as instructed. Eventually we'll get to those others you mentioned.
