Thanks to budler, I have just completed this set but without the checklist that is listed as an Insert on our site. The SCD does not list a checklist card and, considering the few members (3) who have it in their collection, I am wondering of it's origin. I don't think it was a card issued with the other cards inside the package. Was it a cut-out from a Hostess product packaging? Is that why so few are listed in our collections - none being for trade - as the packages were thrown out without the checklist being saved?
The image for it is uploaded by one of our better members so there is no doubt that it exists in his collection. I am hoping that he can enlighten us on the checklist's origin.

Thanks Bob. 11 members have it on their want list but none for trade so if you have some, probably easy trades to do.

The checklist card was on boxes. I wish I saved more than one.

Someday my cards may double in value and then be worth half of what I paid for them.

So you had to cut it out I presume? Looks like many did not. (I knew you would have the answer when I saw you had one. )

UPDATE/CORRECTION:
I found my checklists. 3-card packs came inside the indivual 2-cupcake packages. 6-card packs came inside the 8-count boxes of cupcakes. The checklist card (top image) was not hand-cut, it was the "tray" that the cupcakes sat on in the packages. The box didn't have a checklist card, just a checklist printed on it (bottom image). I bought a lot, A LOT, of individual packages because it was a quicker and cheaper way to get more cards. I wish I had saved more than one checklist card.
