Card Identification Forum

I've tried looking for this card (pictured below) on the TCDB.com checklist and on PSA's website but cannot identify it. The Upper Deck logo on it says "1993" so it is presumably from that year, but I'm wondering what set (if any) it came from and how it was distributed. I've seen unsigned versions of the card, too. Any help identifying this would be appreciated! Thanks!

It was part of a multi-card autograph set (5-7 cards?) available directly from UD Authenticated.

Isn't this the insert from the upper deck series? Similar to the Nolan Ryan and the Hank Aaron, etc.

Yes, 'Career Highlights' is what the cards go by and yes, just Ted was featured. Upon further research, sources claim you could order the cards with or without a signature.
