I have an approx 3.5x5" card that appears to be from this set, numbered '4 of 5'. Is this a missing insert set or am I just having trouble finding it?

Hah, I know its been two years, but I just joined the site and have been adding my collection and found I have the jumbo "3 of 5". Did you ever find an answer to this question? ------------------------------- #1 Lavoy Allen collector.

We just received an IR about these five cards and they definitely exist (here they are on COMC), but I'm having trouble confirming how they were distributed.
